In order to comply with NSSAR requirements concerning the acceptable version of application forms, only SARApAid application form versions 5.xx and later are accepted by the national society at this time.

When using SARApAid or any other NSSAR licensed application program, you must use special blank, watermarked, archival application paper obtained from the NSSAR Merchandise department for the forms which are submitted to the society.  You can order 25 sheets for $6.25 by specifying Cat. No. 0917.
